Q: Anyone use this as a neck knife?

I haven't use it for a neck knife because of the size.

Q: How is this for small of the back horizontal carry ?

It might be a little big for most people; it will conceal pretty good but not very comfortable. Retention could be an issue, too. If you know how to make your own kydex sheaths you might be able to custom make something that would work, but as is you might not like that method of carry.

Q: is the ronin 2 a flat or hollow grind?

It is flat ground and very sharp.
