Does the cupcake maker come with a cookbook or a recipe sheet?

Comes with a very short instruction manual that has basic instructions - to be honest, I never really read it. It is so easy to use - I have made muffins, cupcakes from both scratch and from mixes, and have never had a problem. I use a small ice cream scoop of batter and always use cupcake liners, so there is very little clean up needed.

Does the cupcake plate come out of the maker for easy cleaning?

The cupcake plate does not come out. The cooking plates are coated with non-stick cooking surface, therefore little cleaning is required. Simply wipe cooking plates over with a damp cloth.
