Can this be worn outside a lace-up or other shoe? It looks too bulky to fi tin my shoes without destroying them.

It cannot be worn outside of a shoe. It is somewhat bulky but by far the best ankle brace I have ever used. I am able to wear it with athletic shoes and some boots.

Any idea if you can wear this swimming?

Definitely, I won't swimming with it! Try thinner models.

Do you get a size similar to your shoe size or your sock size?

I wear a size 9.5 men's and it fits perfectly but the sheath ends at mid-point of your foot so actual length and width of your foot isn't that critical. Could easily go up and down a size. Thickness of your bones is relative. I am 5'9" and weigh 160 lbs. medium build.
