Thanks for your question! We always advice all our buyers to use this bed rail only for toddlers as thats what it is made for, however some buyers used them for elderly people and it worked..

Will this work with a really high pillow top mattress?

It is 20 inches from base to top of rail. Depends on how deep your mattress is. If it is more than 10 to 12 inches, it might be iffy.
