No, it will only fit select Fanimation Fans.

Q: are these the blades that slide on over the existing blades? There is only the one picture.

No - they are the blades and they affix right to the base. It's a very attractive fan.

Q: are these blades sold in packs of 5 per order?

Yes, the blades come in a pack of 5.

Q: will they fit the caruso fan, the one with two opposing fan clusters?

Don’t know about Caruso fan. But I had to retrofit and drill holes using old fan blades as a template.
