Is this pen refillable or does it use cartridges?

I have four of them and they are refillable and use cartridges. I like them a lot.

Does the converter have the same weird metal thing that clatters around in the one I use with my Pilot Metropolitan? If y: Any idea of its purpose?

Its and agitator like in a spray paint can to prevent ink from hardening and sticking to the converter.

Does it come with original box or case?
