Honestly, can you wear this under clothes without it being obvious? I'd also be wearing a tee shirt and scrub top and am concerned about overheating

You can wear this under loose fitting clothing, but not under a tight tee shirt. Overheating is a differant story. You can wear this vest while working out; but not more than two hours. The concern wpold be for your skin. The skin needs to sweat to release the heat. One problem with this vest is that its very tight, and does not allow the evaporation needed to cool your skin. I developed a heat rash on my lower chest and upper abdomenthis past summer. Hope thia answer helps.

can you remove the weights?

Yes you can remove the weights, they are in like a pocket and all you have to do is pull the upper part back and take out the weights, you can also buy more weights to add to the vest, more weights the bigger the size can't remember how much for each, you can visit hyperwear.com and it will tell you. It is a very nice comfortable vest. Hope this helps.

Has this been known to help people with autism/sensory processing issues, or is it purely a work out thing?

Yes my son have austim and works great thanks

I am 5'2" and 132 pounds. Should I go for the small or medium? Thank you!

I just got the small Pro and I weigh 119 and am 5' 4" and it's too tight. I wrote Hyperwear support and they said I should have ordered the medium even though the size chart indicates that the small should have fit.
