What insert fits better in the pillow slip, a 16x16 or 15x15?

15X15 would fit better the 16X16 i used was a little tight .

The tag on this item says do not wash, spot clean only. The description on this page says that it is washable. Which one is correct?

Sorry, it was a gift for a 7 year old and I don't think it has needed cleaning yet. (The filler pillow I bought to use with this cover is washable. Think if the stain was bad enough, his mom would gamble on washing the whole pillow--delicate cycle.)
