I see that pets are mentioned but no mention of dosage.  Can anyone tell me the amount based on pet weight etc?

Dosages found on web search:  (Per Day of exposure - for humans and pets):  If using Iosat 130:  
Adults:	One 130 mg tablet once a day.
Adolescents, 12 through 18 years, children 3-12* and Larger Dogs:  65 mg (1/2 Iosat 130 mg), 										
25-30 lbs:  32mg (1/4 Iosat 130 mg tablet)
9-20 lbs:  16mg (1/8 Iosat 10 mg tablet)
If you wish to administer potassium iodide to an animal that has a thyroid (cats, dogs, cattle, horses) follow the dosing information below. Crush it up and put it in their food.   Do not overdose.  Take with food.  Consider the age of the animal. Young animals have the highest risk of health problems from radioactive iodine and radioactive iodine causes long-term potential for thyroid cancer. Therefore, if the animal is elderly there is little chance the animal will get thyroid cancer in its lifetime (one estimate for humans was about 4 years) and elderly pets may have more problems with side effects from the KI.  Note: If a pet has been left on the property during an exposure and the owner is unable to retrieve it before 4 hours then KI will not be effective. If the pet is evacuated with the owner prior to exposure to radiation, there is no need for KI. It is our recommendation that KI only be considered for the pet if the owner has received it and the pet is with the owner.
* Adolescents approaching adult size (> 150 lbs) should receive the full adult dose (130 mg)
* Young children under 3 yrs, and can also be used for small pets - use KI oral solution.  Potassium iodide oral solution is supplied in 1 oz (30 mL) bottles with a dropper marked for 1, 0.5, and 0.25 mL dosing.  Each mL contains 65 mg potassium iodide.

One dose protects the thyroid gland for 24 hours.   Usually a onetime dose is all that is needed.  Emergency officials would notify the public if more dosages are needed.

Never take Potassium Iodide (KI) as a preventive measure.  Only use if radiation exposure is warned from government officials, because although generally safe, use of KI can result in side effects for those with thyroid disorders.  A severe nuclear power plant incident would most likely not be a sudden event, and would probably
take hours or even days to develop. This would enable Federal, State, and County officials to assess
the situation and take necessary public protective actions in a timely manner. To ensure safety at a
nuclear power plant, the concept of “defense in depth” is employed. This means there are several
levels of protection or barriers, each of which is independent of the others. If one should fail, others
would continue to protect the plant, its workers, and the general public. Even if some systems failed,
the remaining safeguards would dramatically slow down the rate of a radioactive release. A nuclear
power plant cannot explode like an atomic bomb. The fuel in a nuclear power plant is too low in
concentration to create the rapid release of energy necessary for an explosion.

Where are these made? i don't care where they're shipped from, where is the country or origin? where were they manufactured and packaged?

IOSat is manufactured by Anbex in VA, you can buy direct from them on their website. Since the situation in Japan there have been numerous reports of counterfeit packages that look identical. Google search " CNN Money fake potassium iodide".
