does this work if the car is off / broken down?

In nearly all cases -- Yes. Since it is plugged into the cigarette lighter, it works as long as the car battery is good. PS. It is really bright. Much better than the rotating one that I used to have.

Where do the fuses go?

I don’t think there is a fuse. It has 50,000 hr life span.

Can I get a replacement lens?

Not sure. May have to reach out to the manufacturer for kens replacement
