Does it have side pockets?

Yes, I bought three of these and they do have pockets on the side.

Would this go over the back of a door?

No, I don't believe it would. I have the model that is exactly as pictured here (with the side pockets), and the hanger parts come out of the top middle and are about 2" inches from the back edge. I can't think of a solution that would physically keep this hanging correctly on the back of the door. If you are able to manage a solution for the hanger part, then yes, the height of this would easily fit on the back of a door.
