Would this work on a jacket with "weather-resistant cotton-poly body" which doesn't have much resistant to water at all?

I believe this is made for synthetic fabrics, specifically for Goretex. If your jacket has a significant blend of cotton (>25-30%), I would suggest you try using a fabric wax (for example: Otter Wax). This WILL change the texture and look of your garment, however, it will add waterproofing and durability.

Must this be used by itself? Or can add it to detergent already loaded in the washing machine?

No, if the garment is not dirty. If it is even slightly dirty, wash with Performance Wash then wash with Clothing Repel in two separate wash cycles.

Can I use this with a top load washer? What are the instructions for use with a top load washer?

I personally use a top load washer, and I put two cups in the top load washer. So far it has worked great for me.
