I bought this some months ago here on Amazon and never recieved any email with instructions?

The construction is in the body of the posting near the bottom of the page.
The wire is divided in three equal pieces twisted together for 1:1 or 9:1 two equal pieces twisted together for 4:1 then placed inside of the heat shrink, it is then wrapped around the ferrite core equal distance turns, 7 turns for 1:1, 8 turns for 4:1, 10 turns for 9:1
The ends of each wire are labeled end to end “A-a” “B-b” “C-c” (Make sure the upper case label and the lower case label are the same wire, use an ohmmeter to check for continuity) 
Then wire up according to what type of balun you want, 1:1 - 4:1 - 9:1

Circular material: nickel zinc ferrite
Magnetic conductivity: 100
Overall dimensions: outer diameter 31 mm * height 7 mm * diameter 18 mm
Center frequency: 15 MHZ
Application frequency: 1-30 MHZ.

Question 3

I have a nice cb mobile antenna that i want to convert to a ground plane antenna with 3, 9ft. wire radials as the plane at 100 w max. will this work?

If you are talking about vertical mobile antenna then a balun most likely isn't required to add radials, these can often be added to normal verticals by changing the mount or modifying it.. Now here comes a question to ask, CB radios (in the USA at least) are limited to 4w AM / 12w SSB so if you are intending to use it for 10m band (vs CB 11m) it might not be the best choice.

Now if you wish to run a 50ohm coax and use a dipole, this is exactly what you want. if you single band/freq you can tune the wires to length otherwise a tuner will be required to multi band @ 100w
