No matter how much i tighten it, the grip still rotates easily like a motorcycle throttle. if i apply any more force, i'll strip the screw for sure.

I had this problem too, first take the grip off the bars, then wipe the part of the bar that the grips clamp on with rubbing alcohol or something, wait for the bar to dry completely, then re-install grips. It would also be a good idea to wipe down the inside of the grip's clamp. After you've done that, tighten the bolt on the clamp. Make sure not to over tighten, as you could strip out the bolt head which would be bad. The grip shouldn't rotate anymore.

Did anyone else have any issues with the end caps not fitting and just sliding out?

tapping end cap with hammer didn't work. The way I got them to correctly seat was by gently pressing on center of cap with blunt handle end of screwdriver. it took about 30lbs of force to slightly deform the end cap which creates space for the cap to properly snap in.
