Do these come a part into individual pieces?

Yes, they come in a box in several different pieces. You put them together yourself.

The product images are inconsistent. Are these 3 screw holes or 4?

Mine have 3 holes each, 3 holes on the part that screws into the wall, and 3 holes on the part that screws into the shelving. They are beautiful and sturdy.

Do you have to put this in a double wall stud because of four screws?

I'm no handyman expert, but if you get 2 screws into a stud, it should hold quite steady. You could leave the other holes open or put a screw(s) in for looks of your choice.

what size are the pipes?

I did measure it. 1/2 in pipe (outer diameter about 3/4 in.) I'm sorry because I know you specifically asked months ago. I want to post a picture but Amazon's only letting me post a video? Tech glitch. But it's definitely 1/2 inch pipe (measured by inner).
