Why does the price suddenly change $12 to $19?

that depends on the seller, and the demand of the product

What kind of box does it come in ?

I don't recall a box of any kind.

Does this item have a red underneath visor as shown in the pictures? I ordered before and I received an all blue hat

The blue one that I received does not have a red under the brim.

It's adjustable. It's one size, then you adjust it like you want it.
