It’s pretty good, a good few hours, obviously it depends what you are doing and how wrapped up you are.

If it fits inside, yes. I wouldn't force a bigger hot/cold pack in, though. Just remember if the pack you use isn't exactly the same size it will move around inside the pouch and you probably won't get the full benefit.
