Does this need to be refrigerated after opening?

No, I didn't. But I used it up pretty quickly, in coffee, and on ice cream. I wouldn't leave it open, unrefrigerated for many months.

Is this like the Dunkin Donuts caramel swirl?

I have tried many caramel sauces and none are very similar to Dunkin caramel swirl so don't get your hopes too high. That being said, Torani is still probably the best alternative I have found
