After hoisting an item up, can you control the lowering speed with the handle? Or does it spin the spool but not handle when released?

The handle is permanently engaged with the spool gear. Therefore, you can control the speed you crank it out. But, beware if it gets away from you it will spool out uncontrollably.

Is this safe to remain under load, as if mounted in the garage to hold a 150lb item at ceiling level for months at a time?

We use it to hold up a corn feeder with 100lbs of corn. It's been under load and in the elements for 6 months and looking good. I see no problem as long as the mounting hardware you use can support the load.

What bolt is needed for strap attaching?

I used a 1/4 x 2-1/2 partially threaded (not fully threaded) hex head bolt with a nylon lock nut. The threads on the bolt do come into contact with the strap a little on the edge, so I filed them down a little and then filled them with silicone. I think there may be a bolt with less threading but This is what I had and it works great. It is 316 stainless so hopefully no corrosion.
