How do you know that it needs to be changed

You need to check the water with silver nitrate and if chlorine present will go cloudy but about every 2 years I have found dependant on amount used.

I use this item for my fish tanks and wanted to know if it takes to much minerals out of the water for the fish?

I have used this at home and check the water quality regularly and find it no problem. At work I use it for all 50 large fish tanks both marine, tropical and cold water as well as amphibians and water based reptiles with no problems

does this filter also remove nitrates and phosphates?

The purpose of this device is to remove chlorine from the tap water filling the pond. It won't remove toxins in the water such as ammonia, nitrite and nitrate, you'll need a mechanical/biological filter system to do this.

How is this compared to a 3 stage dechlorinator?

Expensive very fragile. Not good outdoors if frost expected. To be frank I have now purchased a large single stage filter for topping up pond. It’s convenient if you just want to connect to a hose and store afterwards. Be careful though I used mine twice and broke it on the patio.
