Does this work for Cello players?

It can work for any instrument and any key. Both top and bottom parts of the ring spin so you are able to change it to what is necessary for you. If you look at the ring it works with the circle of fifths.

are these sizes a universal ring sizing as in the jewelry stores?

Pretty close, but be aware that the ring is thick, broad and heavy and is likely to fit a bit more snugly than an ring with a more "normal" band. I bought the biggest and it fits my middle finger, but I mostly wear it on a stainless steel chain around my neck. It's easier to use that way and to show to others. I wouldn't put it on a sterling chain--too heavy and steel would destroy silver.

can anyone explain how this ring uses the cicrle of 5ths? I was expecting it to have the notes arrange in the circle of 5th order, not chromatic?

You have to set it to whatever degree of transposition you want. If you want fifths, match up C with G (or F to go the other direction).
