Does it need to be diluted or ready to go as it is

It definitely should be diluted. The strength you use depends on the soiling or staining of the fabric you are working on.

Can this be used to clean car seat foam that has the leather cover removed with urine odor? I want to submerge them in water and carpet bomber.

Angry Orange concentrate is the best. It's expensive but it works. I use it on wicket smells like insoles, wet mouldy smelling sneakers and hiking boots. For bad smells like yours I mix about an ounce or two in a large spray bottle. Once the smell was so bad I used full strength. It's much better than lemon and the only think that works and keeps the smell from coming back. Put it on and let it soak rinse with clean water warm and lightly respray and let it dry. It is not soapy so it rinses clean the first time.

Can I use this on Alcantara seats ?

We would recommend not using Carpet Bomber on Alcantara seats

Is the 16oz version the same concentration as the gallon or is it already pre-diluted either 5 or 8:1?

Carpet Bomber can be used as straight or diluted at 1:4 depending how bad the surface that You are using it on. I personally dilute it 1:4 from the gallon
