All about me/child record forms permissions book

I have purchased this twice now. Brilliant, child records in there, but 1 thing missing, space for 3 contact numbers for emergency on the record. Otherwise saves on paperwork.

Ordered this as starting out, showed a childminder who has been doing it 10 years and she ordered them. Easy to photocopy

I’m a Registered childminder, I decided to give Amazon a go for paperwork for my business, the form wasn’t exactly what I was expecting and it was poorly printed on poor quality paper! Will not be using Amazon for my business again!
