Is there a pad on just the bottom or top and bottom?

Just the bottom and it fell out after a week of use - NOT the Revlon quality as usual :( The finger grips fell out after the second use.

Does it come with a replacement rubber piece?

Yes it does. It comes with 1 replacement piece.

Where can I get replacement pads for this curler?

I think the eyelash curler came with an extra one and I also bought more than one eyelash curler. I do think they sell this kind at Kmart and maybe walgreens though hope this helps ð~
